Summary
This study reports the first case of transretinal membrane formation in diffuse unilateral subacute neuroretinitis (DUSN). Surgical removal of this membrane improved visual acuity in a patient with DUSN.

Background:
- Diffuse unilateral subacute neuroretinitis (DUSN) presents with unilateral vision loss, vitreous inflammation, optic disc swelling, and retinal lesions, often linked to nematode infection.
- Transretinal membrane formation has not been previously reported in DUSN cases.
Observation:
- A 22-year-old woman presented with decreased vision and neuroretinitis, diagnosed as DUSN.
- A subretinal nematode was identified and successfully eradicated using laser photocoagulation.
- A persistent epiretinal membrane, noted to be transretinal, required surgical removal via pars plana vitrectomy.
Findings:
- Laser treatment effectively destroyed the nematode, leading to partial resolution of neurosensory detachment and improved visual acuity.
- Transmission electron microscopy revealed the membrane composed of a hypocellular collagenous stroma with fibroblasts and inflammatory cells.
Implications:
- This case highlights a novel complication of DUSN: transretinal membrane formation.
- Surgical intervention for macular-involved membranes may offer visual benefits for select DUSN patients.
