Purpose of the Study:

  • To evaluate the utility of intraoperative angioscopy for examining coronary artery bypass graft anastomoses.
  • To correlate angioscopic findings with graft flow, angiographic data, and early postoperative outcomes.

Main Methods:

  • Seventy-five distal anastomoses and vein grafts were examined using 0.8- to 2.5-mm fiberoptic angioscopes.
  • Angioscopic findings were compared with angiographic data, vessel morphology, and graft flow.
  • Examinations were performed with clear cardioplegia irrigation and took under 2 minutes.

  • Satisfactory images were obtained in 72 of 75 anastomoses.
  • Minor angioscopic abnormalities (intimal flaps, thrombus, irregularities) were found in 17 anastomoses (24%) but did not require revision.
  • Low graft flow was correlated with vessel size and runoff, not with observed angioscopic abnormalities.
  • Intracoronary angioscopy showed discrepancies with angiography in 4 of 5 cases.

Conclusions:

  • Intraoperative angioscopy is a safe and efficient tool for assessing coronary artery bypass graft anastomoses.
  • Minor angioscopic abnormalities are common but do not appear to compromise early graft patency.
  • Angioscopy provides complementary information to angiography in evaluating bypass graft quality.
