Background:

  • A newly described histopathological finding, the glomerular tip lesion, is characterized by specific structural changes within the glomerulus.
  • This lesion is localized to the peripheral segments of the glomerular tuft.

Observation:

  • The lesion features focal and segmental mesangial matrix widening without hypercellularity.
  • Capillary dilatation, hyaline exudation, and vacuolized cells are observed within the capillary walls.

Findings:

  • Clinically, patients exhibit pronounced nephrotic syndrome and mildly reduced renal function.
  • Despite the presence of the lesion, terminal renal failure is seldom observed even over extended periods.

Implications:

  • The precise classification of glomerular tip lesions within kidney disease categories is currently uncertain.
  • It may represent a transitional stage between minor lesion nephritis and focal segmental sclerosis, or a distinct entity related to high proteinuria.
