Background:

  • Osteoarthritis significantly impacts quality of life in the elderly.
  • Total knee arthroplasty (TKA) is a common treatment for knee osteoarthritis.
  • Limited data exists on TKA outcomes specifically in the very aged population (85+ years).

Observation:

  • Twenty-six TKAs were performed on 20 patients aged 85+ years, with 20 TKAs in 15 patients available for 3-year follow-up.
  • The primary indication for surgery was pain control.
  • Postoperative outcomes were assessed using a modified Knee Society scoring system.

Findings:

  • Nineteen of 20 knees reported good or excellent pain relief.
  • Quality of life improved for most patients.
  • Eleven knees received excellent, 4 good, 2 fair, and 3 poor ratings.
  • No perioperative mortality or wound healing complications were observed.

Implications:

  • Total knee arthroplasty is a safe and reliable option for pain and deformity management in very elderly patients with knee arthritis.
  • The procedure can significantly improve pain and quality of life in this challenging patient group.
  • Careful patient selection and management are crucial for optimal outcomes in geriatric TKA patients.
