Background:

  • Hereditary multiple exostosis (HME) is a rare genetic disorder characterized by the development of multiple bony outgrowths.
  • Complications of HME are varied, but hemothorax is an exceptionally rare presentation.

Observation:

  • A 12-year-old boy with known HME presented with acute thoracic pain and pleural effusion.
  • Initial conservative management failed, and thoracentesis revealed hemothorax.
  • Imaging confirmed intra-thoracic costal exostoses as the likely cause.

Findings:

  • The hemothorax resolved spontaneously within 12 days following thoracentesis.
  • Bacteriological and cytologic analyses were negative, ruling out infection or malignancy.

  • Implications:

    • Internal costal exostoses should be considered in the differential diagnosis of hemothorax in patients with HME.
