Summary

Interaural time discrimination (ITD) testing can help diagnose retrocochlear lesions, especially when brainstem auditory evoked potentials (BAEPs) are unclear. Normal ITD thresholds suggest a low probability of retrocochlear damage.

Area of Science:

  • Neuroscience
  • Auditory Neuroscience
  • Clinical Audiology

Context:

  • Interaural time discrimination (ITD) is crucial for sound localization.
  • Deficits in binaural timing are linked to brainstem auditory evoked potentials (BAEPs) abnormalities in multiple sclerosis.
  • The relationship between ITD and BAEPs in other otoneurological conditions requires investigation.

Purpose:

  • To correlate psychophysical ITD thresholds with BAEPs in patients with otoneurological deficits.
  • To assess the diagnostic utility of ITD testing in conjunction with BAEPs.

Summary:

  • ITD thresholds and BAEPs were measured in 168 patients.
  • Both tests were normal in 29.8% and abnormal in 42.2% of cases.
  • Discrepancies (ITD abnormal/BAEPs normal or vice versa) were observed in 28.0% of patients, with ITD abnormal/BAEPs normal being more common (23.2%).

Impact:

  • Normal ITD thresholds indicate a low probability of retrocochlear lesions.
  • ITD testing serves as a valuable tool when BAEP interpretation is challenging due to desynchronized waves.
  • This study highlights the complementary roles of ITD and BAEPs in diagnosing auditory pathway disorders.
