Background:

  • Osteosarcoma is a primary bone cancer often requiring aggressive surgical treatment.
  • Limb preservation strategies are increasingly important in oncologic surgery.
  • Human leucocyte interferon (hLI) has been explored as an adjuvant therapy.

Observation:

  • Fifteen patients with primary classical osteosarcoma underwent en bloc resection and adjuvant hLI therapy.
  • Long-term follow-up exceeding fifteen years was conducted for all patients.
  • Treatment allowed for limb preservation as an alternative to ablative surgery.

Findings:

  • 50% of patients demonstrated no evidence of tumor recurrence after more than 15 years.
  • Eight patients succumbed to generalized osteosarcoma disease.
  • Two patients developed distant lung metastases, diagnosed 5 and 8 years post-surgery.

Implications:

  • Limb-sparing surgery combined with adjuvant hLI offers a viable alternative for select osteosarcoma cases.
  • This multimodal approach may improve long-term outcomes in osteosarcoma patients.
  • Specialized centers with expert surgical teams are crucial for successful implementation of this treatment strategy.
