Background:

Benign symmetric lipomatosis (BSL) is a rare condition characterized by symmetrical, non-encapsulated adipose tissue deposition.
It typically affects middle-aged men and is often associated with alcohol abuse, though idiopathic cases exist.

Observation:

A case of massive BSL is presented, notable for its significant size and association with laryngeal compression.
The lipomatosis caused airway compromise due to the mass effect on the larynx.

Findings:

The benign nature of the lipomatous tissue was confirmed histologically.
Laryngeal compression was the primary clinical manifestation, leading to respiratory distress.
