Summary

Clozapine effectively treats severe mood disorders, especially manic phases of schizoaffective and bipolar disorders. It shows good tolerability, even for patients unresponsive to other treatments.

Purpose of the Study:

  • To evaluate clozapine's efficacy and tolerability in severe mood disorders.

Main Methods:

  • Systematic review of MEDLINE-identified studies.
  • Inclusion criteria: schizophrenia, bipolar disorder, schizoaffective disorder, major depression, organic disorders with psychotic or major affective syndromes.

Main Results:

  • Patients with manic or psychotic phases of schizoaffective or bipolar disorder showed significantly higher response rates to clozapine compared to schizophrenia (p = .006) or severe depressive syndromes (p = .001).
  • No significant differences in clozapine's side effect profile were observed between patients with severe mood disorders and those with schizophrenia.

Conclusions:

  • Clozapine demonstrates effectiveness and good tolerability for short-term and maintenance treatment of severe or psychotic mood disorders.
