The quadrilateral space syndrome

Summary

Quadrilateral space syndrome can cause shoulder pain, especially with arm movement. This condition involves the posterior circumflex humeral artery and should be considered in unexplained shoulder pain cases.

Background:

Shoulder pain is a common complaint, often with multifactorial causes.
Desk work and specific arm positions can exacerbate certain shoulder pathologies.
Vascular compression syndromes are increasingly recognized in musculoskeletal conditions.

Observation:

A 21-year-old female presented with left shoulder pain aggravated by desk work.
Left subclavian angiography was performed with the arm in abduction and external rotation.
Angiography revealed occlusion of the posterior circumflex humeral artery.

Findings:

The observed arterial occlusion is consistent with the diagnostic criteria for quadrilateral space syndrome.
This case highlights a specific vascular compromise linked to anatomical positioning.
The posterior circumflex humeral artery is implicated in the pathophysiology of this syndrome.

Implications:

Quadrilateral space syndrome should be included in the differential diagnosis for patients with idiopathic shoulder pain.
Early recognition and diagnosis can prevent chronic pain and functional limitations.
Further research into the biomechanics and management of this syndrome is warranted.
