Background:

  • Degenerative cervical spine disease can lead to dynamic spinal canal stenosis.
  • Standard MRI may not detect stenosis that only occurs during specific neck movements.

Purpose of the Study:

  • To evaluate functional stenosis of the cervical spine using kinematic MRI.
  • To compare findings in healthy volunteers versus patients with degenerative cervical spine disease.

Main Methods:

  • Kinematic MRI of the cervical spine was performed from 50° inclination to 30° reclination.
  • Sagittal T2* weighted gradient echo sequences were acquired at 9 equal angle positions.
  • 23 healthy volunteers and 23 patients with degenerative disease were included.

  • Healthy volunteers showed physiological narrowing (up to 50%) at inclination and widening (up to 10%) at reclination.
  • Functional spinal canal stenosis with spinal cord compression occurred in 22% of patients at inclination and 65% at reclination.
  • No change in stenosis was found in 13% of patients.

Conclusions:

  • Kinematic MRI identified functional spinal canal stenosis and myelon compression in patients with degenerative cervical spine disease.
  • These findings were not apparent on standard MRI.
  • Kinematic MRI is recommended for early detection of functional myelon compression and therapeutic planning.
