Area of Science:

  • Pharmacology
  • Cardiovascular Medicine
  • Clinical Therapeutics

Background:

  • Idiopathic orthostatic hypotension and associated symptoms like neuroticism can significantly impact quality of life.
  • Low blood pressure (below 100/60 mmHg) can manifest with various non-specific symptoms, necessitating effective treatment options.

Purpose of the Study:

  • To evaluate the efficacy and safety of Midodrine (Gutron) in women with chronic low blood pressure and associated symptoms.
  • To assess the impact of Midodrine on hemodynamic parameters, biochemical markers, and endothelial function.

Main Methods:

  • An 84-day open-label study involving 8 women (average age 34.5 years) with blood pressure below 100/60 mmHg.
  • Comprehensive assessments included clinical examinations, ECG, Holter monitoring, Doppler pressure measurements, EEG, and various blood and urine tests.

Related Experiment Videos

  • Specific tests like the Jarmatz-de Marée test were used to evaluate orthostatic responses.
  • Main Results:

    • Midodrine (Gutron) demonstrated no adverse side effects and resulted in sustained subjective improvements in all patients.
    • Significant increase in upright diastolic blood pressure was observed, particularly during prolonged standing.
    • No adverse changes were noted in biochemical parameters, ECG, Holter monitoring, pulse rate, Doppler index, or EEG.
    • Midodrine did not affect platelet aggregation or cause toxic effects on vascular endothelium.

    Conclusions:

    • Midodrine (Gutron) is a safe and effective treatment for women with chronic low blood pressure, improving both hemodynamic status and subjective well-being.
    • The sustained improvement and lack of side effects suggest Midodrine as a valuable therapeutic option for this patient group.
