Post-irradiation sarcoma is a rare complication of radiation therapy for soft tissue sarcoma. Early detection and wide margin resection are crucial for successful treatment of these secondary neoplasms.

Area of Science:

  • Oncology
  • Radiation Oncology
  • Surgical Oncology

Background:

  • Soft tissue sarcomas are malignant tumors arising from connective tissues.
  • Radiation therapy is a common treatment modality for soft tissue sarcomas.
  • Post-irradiation sarcoma is a rare but serious complication following radiation treatment.

Purpose of the Study:

  • To investigate the incidence, characteristics, and outcomes of patients who developed post-irradiation sarcoma after treatment for soft tissue sarcoma.
  • To emphasize the importance of early detection and surgical management of these secondary malignancies.

Main Methods:

  • Retrospective review of 375 patients treated for soft tissue sarcoma between 1975 and 1993.
  • Analysis of patients who developed a second neoplasm, specifically post-irradiation sarcoma.

Related Experiment Videos

  • Detailed review of radiation dosage, time interval to sarcoma development, tumor characteristics, staging (CT/MRI), surgical treatment, and patient follow-up.
  • Main Results:

    • 11 out of 375 patients (2.9%) developed post-irradiation sarcoma.
    • Mean time from initial radiation to sarcoma onset was 15.8 years (range 4-31 years).
    • Mean radiation dose was 40 Gy (range 12-60 Gy). Common types included malignant fibrous histiocytoma and haemangiosarcoma.
    • Surgical resection with wide margins was achieved in 10 of 11 patients, with only 1 death due to the tumor after a mean follow-up of 4.7 years.

    Conclusions:

    • Post-irradiation sarcomas, though infrequent, should be suspected in patients with new symptoms or changes in previously irradiated areas.
    • Prompt diagnosis and aggressive surgical resection with wide margins offer a chance for curative treatment.
    • Continued vigilance and follow-up are essential for patients treated with radiation for soft tissue sarcoma.
