Summary

Systemic lupus erythematosus (SLE) is rare in elderly males. This study highlights two cases, suggesting SLE should be considered in older men with multiple organ issues, with IVIG showing promise as an alternative treatment.

Background:

  • Systemic lupus erythematosus (SLE) is an autoimmune disease with a predilection for women, rarely diagnosed in elderly males.
  • Clinical presentation in elderly males can be atypical, potentially delaying diagnosis.

Observation:

  • Two male patients over 65 presented with anemia, cytopenia, nephritis, and serositis, initially suggesting an autoimmune condition.
  • Diagnosis of SLE was established after 6 and 9 years, respectively.
  • Both patients exhibited hypothyroidism and ocular manifestations (uveitis, iridocyclitis), indicative of broader autoimmune activity.

Findings:

  • One patient experienced significant side effects from corticosteroid and azathioprine therapy.
  • The other patient showed stable disease for 18 months on intravenous immunoglobulin (IVIG) therapy.
