Summary

Hyperventilation can cause tingling and cramps, but this study found they are not linked to changes in ionized serum calcium levels. Other electrolytes, except sodium, did show significant alterations during hyperventilation.

Background:

  • Respiratory alkalosis from hyperventilation is commonly associated with paresthesia and tetanic finger cramps.
  • This is traditionally attributed to a pH-dependent reduction in ionized serum calcium.

Purpose of the Study:

  • To investigate the relationship between ionized serum calcium levels and symptoms of paresthesia and tetanic finger cramps during induced hyperventilation.
  • To examine changes in other serum electrolytes during hyperventilation.

Main Methods:

  • Ten healthy volunteers underwent a three-minute hyperventilation period.
  • Measurements included ionized serum calcium, total calcium, total protein, and other electrolytes (sodium, magnesium, potassium, chloride, phosphate, bicarbonate).

  • All volunteers experienced paresthesias during hyperventilation.
  • No significant change in ionized serum calcium was observed (1.26 +/- 0.05 mmol/l).
  • Total protein increased due to hemoconcentration; significant changes were noted in most electrolytes except sodium.

Conclusions:

  • Paresthesia and tetanic finger cramps during hyperventilation are not directly caused by a decrease in ionized serum calcium.
  • Changes in other electrolytes, excluding sodium, play a role in the observed symptoms during respiratory alkalosis.
