Background:

Family studies are crucial for understanding schizophrenia's genetic basis.
Schizophrenia spectrum diagnoses include relatives of probands with related disorders.

Observation:

Fifty relatives of schizophrenia probands were analyzed.
Eleven relatives received diagnoses of nonaffective psychotic disorder or personality disorders.
Diagnostic dilemmas arose from classifying delusional phenomena.

Findings:

Unusual beliefs presented classification challenges: odd vs. delusional, transient vs. persistent, bizarre vs. non-bizarre.
Delusions may represent continuous dimensions rather than distinct categories.
Four complex cases illustrating these diagnostic issues are presented.

Implications:

Revising diagnostic criteria for delusions could improve classification accuracy.
Understanding these diagnostic nuances is vital for genetic linkage studies of schizophrenia.
Continuous dimensional approaches to delusions may offer new insights into schizophrenia's etiology.
