Background:

  • Non-Hodgkin's lymphoma (NHL) is a heterogeneous group of lymphoid malignancies.
  • Bone marrow (BM) involvement is a critical prognostic factor in NHL.
  • Understanding the patterns and incidence of BM involvement is essential for accurate staging and treatment planning.

Purpose of the Study:

  • To evaluate the incidence and patterns of bone marrow involvement in patients with non-Hodgkin's lymphoma at diagnosis.
  • To compare the diagnostic accuracy of different bone marrow examination methods.

Main Methods:

  • Retrospective study of 156 patients diagnosed with non-Hodgkin's lymphoma.
  • Analysis of bone marrow biopsy, smear, and clot sections for lymphoma involvement.
  • Histopathological classification and grading of lymphoma subtypes.

Main Results:

  • 35% of NHL patients showed bone marrow involvement at diagnosis.
  • Diffuse large cell lymphoma was the most common type with BM involvement (30%).
  • Interstitial and diffuse patterns were most frequent, with extensive marrow space involvement (>76%) in 42% of cases. Bone marrow biopsy demonstrated higher sensitivity (82%) compared to smear (72%) and clot section (69%).

Conclusions:

  • Diffuse large cell lymphoma is the most common histologic type with bone marrow involvement.
  • Interstitial and diffuse patterns, along with extensive marrow space involvement, are frequent.
  • Bone marrow biopsy is the preferred method for detecting lymphoma involvement in the marrow.
