Background:

  • Granulocytic sarcoma (GS) is a solid tumor of immature granulocytic precursors in extramedullary sites.
  • GS is associated with various hematologic malignancies, most commonly acute non lymphoblastic leukemia (ANLL), myelodysplastic syndromes (MDS), and chronic myeloid leukemia (CML) in blastic crisis.

Observation:

  • A retrospective study reviewed five cases of GS diagnosed over nine years.
  • Tumor locations varied, including lymph nodes, jaw, paravertebral region, gallbladder, and retroperitoneum.
  • Patients presented with ANLL or refractory anemia with excess of blasts (RAEB), with GS appearing at diagnosis, relapse, or preceding ANLL.

Findings:

  • Conventional cytologic and anatomopathologic studies can suggest GS, but histochemical and immunohistochemical staining are crucial for definitive diagnosis.

    • Immunocytochemical and immunohistochemical techniques are invaluable for differentiating GS from other tumors, especially anaplastic non-Hodgkin lymphomas.
    • The aggressive nature and poor prognosis of GS highlight the need for early and accurate diagnosis.
