Summary

Spinal vascular malformations can mimic spinal cord tumors in adults. These rare caudal lesions may present as isolated intramedullary masses with normal-appearing surface vessels, complicating diagnosis.

Background:

  • Progressive caudal spinal cord disease can present with subacute symptoms in adults.
  • Diagnostic imaging, including CT, MRI, and myelography, is crucial for evaluating spinal cord lesions.

Observation:

  • Three adult cases of progressive caudal spinal cord disease were initially diagnosed as spinal cord neoplasms via imaging.
  • Neuroimaging and intraoperative assessments revealed no abnormal surface vessels in these cases.

Findings:

  • Histopathological examination of biopsy specimens confirmed spinal vascular malformations in all three patients.
  • These vascular malformations appeared as isolated intramedullary lesions.

Implications:
