Summary

Laparoscopy reliably diagnoses and manages nonpalpable testes in children, guiding surgical strategies. This approach allows for effective treatment, including staged orchidopexy for intraabdominal testes and exploration for canalicular testes.

Area of Science:

  • Pediatric Surgery
  • Urology
  • Diagnostic Imaging

Background:

  • Nonpalpable testes present diagnostic and therapeutic challenges in pediatric surgery.
  • Conventional imaging methods (ultrasound, CT, MRI, vasography, herniography) lack complete reliability in locating testes or confirming their absence.

Purpose of the Study:

  • To evaluate the diagnostic and therapeutic utility of laparoscopy in managing nonpalpable testes in pediatric patients.
  • To correlate laparoscopic findings with subsequent surgical management strategies.

Main Methods:

  • A preliminary experience with laparoscopy in 30 patients (age 2-5 years) with 34 nonpalpable testes was reported.
  • Laparoscopic findings determined operative steps, including orchiectomy, testis detorsion, internal spermatic vessel clipping, Fowler-Stephens orchidopexy, and inguinal exploration.

Related Experiment Videos

  • Follow-up included staged procedures and assessment of relocated testes size and trophism.
  • Main Results:

    • Laparoscopy accurately located 18 intraabdominal, 7 canalicular, and 9 atrophic/absent testes.
    • Intraabdominal testes management included orchiectomy (2), detorsion (1), and staged orchidopexy (15).
    • Inguinal exploration revealed 4 orchiectomies and 3 standard orchidopexies; 9 testes were confirmed absent. Staged orchidopexy showed good results after 8-10 months.

    Conclusions:

    • Laparoscopy is a highly reliable tool for diagnosing and managing nonpalpable testes in children.
    • Laparoscopic findings effectively guide surgical strategy, optimizing outcomes for various presentations.
    • Testicular prosthesis insertion is recommended for absent/vanishing testes to ensure adequate scrotal development.
