Background:

Gallbladder anatomical variants are common, but duplication anomalies are rare.
These anomalies present diverse configurations based on lobe fusion and cystic duct arrangement.

Observation:

A case of a deeply cleft, bilobed gallbladder was visualized using computed tomography (CT), ultrasonography (US), and oral cholecystography (OCG).
The anomaly featured complete duplication of the gallbladder body and fundus into two separate lobes, sharing a single infundibulum.

Findings:

Detailed imaging confirmed a rare congenital gallbladder duplication anomaly.
The bilobed gallbladder demonstrated complete separation of the body and fundus with a unified infundibulum.
