Summary

Clofazimine, a medication for erythema nodosum leprosum, may cause severe gastrointestinal issues and potentially fatal cardiac arrhythmias like torsade de pointes. Discontinuation of clofazimine resolved the patient's symptoms.

Area of Science:

  • Pharmacology
  • Cardiology
  • Dermatology

Background:

  • Clofazimine is a key treatment for erythema nodosum leprosum (ENL).
  • Long-term clofazimine use can have various side effects.

Observation:

  • A 66-year-old male on clofazimine for 11 months developed severe gastrointestinal upset.
  • He subsequently experienced ventricular tachycardia, suggestive of torsade de pointes.
  • The arrhythmia persisted despite magnesium correction and lidocaine treatment.

Findings:

  • Gastrointestinal symptoms resolved after clofazimine discontinuation.
  • Patient exhibited keratopathy and myelin figures in white blood cells, similar to amiodarone or chloroquine toxicity.
  • Clofazimine was postulated as the cause of cardiac arrhythmia, possibly exacerbated by electrolyte imbalance.

Related Experiment Videos

Implications:

  • This case highlights potential cardiotoxicity of clofazimine, necessitating careful patient monitoring.
  • Clinicians should consider clofazimine as a potential cause of unexplained arrhythmias in ENL patients.
  • Further research into clofazimine's cardiac side effect profile is warranted.
