Summary

This study found that mitochondrial dysfunction is common in patients with chronic progressive external ophthalmoplegia, generalized muscle atrophy, and myalgia. These findings suggest mitochondrial disorders may play a significant role in these myopathies.

Background:

  • Chronic progressive external ophthalmoplegia (CPEO) with adult-onset generalized muscle atrophy and myalgia is a rare condition.
  • Histological examination can reveal centronuclear myopathy or fiber-type disproportion.
  • Ragged red fibers and cytochrome c oxidase deficiency are indicative of mitochondrial abnormalities.

Observation:

  • Three patients with CPEO, muscle atrophy, and myalgia presented with ragged red fibers and subsarcolemmal mitochondrial accumulation.
  • Muscle biopsies showed cytochrome c oxidase negative fibers and increased subsarcolemmal mitochondria.
  • Patients underwent comprehensive investigations including biochemical, genetic, and spectroscopic analyses.

Findings:

  • Biochemical analysis of the respiratory chain suggested additional mitochondrial dysfunction in all patients.

  • Molecular genetics and magnetic resonance spectroscopy further supported the presence of mitochondrial impairment.
  • Ergometry results were consistent with an underlying mitochondrial disorder.
