Six patients experienced severe, short-lasting ice-pick headaches in the scalp

Area of Science:

  • Neurology
  • Headache Medicine

Background:

  • Idiopathic stabbing headache (ISH) is characterized by brief, severe head pain.
  • ISH typically affects the trigeminal nerve distribution.

Observation:

  • Six patients presented with daily, ice-pick-like headaches lasting about a week.
  • Pain occurred in the retroauricular, parietal, and occipital scalp regions, outside the trigeminal nerve area.
  • Patients had no prior history of migraine.

Findings:

  • This headache presentation differs from ISH due to its "status" profile, extratrigeminal posterior location, and lack of migraine association.
  • Despite severity and recurrence in some, the condition was self-limited and benign.
  • All patients responded promptly to indomethacin treatment.

Implications:

  • This case series suggests a distinct headache entity potentially treatable with indomethacin.
  • Recognition of this extratrigeminal stabbing headache is important for accurate diagnosis and management.
  • Further research may clarify the pathophysiology and optimal treatment strategies for this headache type.
