Background:

  • Primary lung melanoma is an exceptionally rare condition.
  • Distinguishing primary lung melanoma from metastatic disease is diagnostically challenging.

Observation:

  • A 66-year-old woman presented with cough and hemoptysis.
  • Bronchoscopy revealed a polypoid mass obstructing the right lower bronchus.
  • Histopathology confirmed malignant melanoma, positive for S-100 protein and HMB45.

Findings:

  • The diagnosed bronchial tumor was confirmed as primary malignant melanoma.
  • Extensive investigations ruled out other potential primary tumor locations.
  • The case meets criteria for definite primary melanoma of the lower respiratory tract.

Implications:

  • This case contributes to the limited literature on primary bronchial melanoma.
  • Accurate diagnosis is crucial for appropriate patient management and prognosis.
  • Highlights the importance of immunohistochemistry in differentiating primary lung tumors.
