[Comparative characteristics of steroidogenesis and its biorhythm in the hyperfunctioning adrenal cortex]

    Summary

Daily steroid hormone rhythms in Cushing's disease (CD) and congenital adrenal hyperplasia (CAH) were studied. Some CD patients and most CAH patients showed normal rhythms, but enzyme activities differed, indicating distinct disease mechanisms.

    Area of Science:

    • Endocrinology
    • Steroid biochemistry

    Background:

    • ACTH-dependent Itsenko-Cushing's disease (CD) and congenital adrenal hyperplasia (CAH) are endocrine disorders affecting steroid hormone production.
    • Understanding daily rhythms of steroid hormones is crucial for diagnosing and managing these conditions.

    Purpose of the Study:

    • To investigate and compare the daily rhythms of steroid hormones in patients with CD and CAH.
    • To analyze enzymatic activities involved in steroidogenesis in both patient groups.

    Main Methods:

    • Analysis of daily rhythms of adrenal C21- and C19-steroids, ACTH, and testosterone.
    • Assessment of steroid hormone precursor ratios to infer enzymatic activities (17-, 11 beta-, 18-, and 21-hydroxylases).

    Main Results:

    Related Experiment Videos

  • Normal daily steroid hormone and ACTH rhythms were observed in 23.5% of CD patients.
  • CAH patients exhibited marked daily rhythms of adrenal androgens and testosterone, similar to cortisol rhythms.
  • Ratios indicated enhanced 17-, 11 beta-, and 18-hydroxylase activities in CD, with normal activities in CAH, except for 21-hydroxylase.
  • Conclusions:

    • Steroid hormone rhythmicity varies in CD, with a subset showing normal patterns.
    • CAH patients generally maintain typical daily steroid hormone rhythms.
    • Enzyme activity profiles suggest distinct pathogenic mechanisms in CD and CAH, highlighting specific hydroxylase involvement.
