Summary

Acute hyperbaric oxygenation (HBO) exposure can induce retinopathy of prematurity (ROP) in newborn rats by causing retinal vasoconstriction and subsequent vascular proliferation. Hypercapnia partially mitigated this effect, but did not fully prevent ROP development.

Background:

  • Retinopathy of prematurity (ROP) is typically linked to prolonged normobaric hyperoxia in premature infants.
  • The potential for acute hyperbaric oxygenation (HBO) to induce ROP has not been extensively studied.

Purpose of the Study:

  • To investigate if acute HBO exposure can induce experimental ROP in newborn rats.
  • To test the hypothesis that retinal vasoconstriction during HBO is a key factor in ROP development.

Main Methods:

  • Newborn rats were exposed to 5 hours of either HBO (5 atm abs O2) or HBO with hypercapnia (5 atm abs O2 + 190 mmHg PCO2).
  • Control rats breathed air at atmospheric pressure.
  • Retinal vascular density (RVD) was quantified using India ink perfusion and image analysis two months post-exposure.

Main Results:

  • HBO exposure significantly increased RVD compared to controls (0.0417 vs. 0.0112).
  • HBO with hypercapnia showed a non-significant increase in RVD, reducing the HBO-induced RVD increase by 39%.

Conclusions:

  • Sustained HBO-induced retinal vasoconstriction followed by hypoxic-ischemic injury may initiate ROP development in newborn rats.
  • Hypercapnia partially reduces HBO-induced retinal vasoproliferation but does not completely prevent it, likely due to increased tissue PO2 and oxidative damage.
