Background:

  • Previous pilot studies indicated the feasibility and safety of outpatient marrow harvesting.
  • Hematologic malignancies often require hematopoietic stem cell transplantation, necessitating marrow harvesting.

Purpose of the Study:

  • To analyze and compare outcomes of outpatient versus inpatient marrow harvests.
  • To confirm the safety and efficacy of outpatient marrow harvesting procedures.

  • Retrospective analysis of 149 outpatient marrow harvests.
  • Comparison with 61 inpatient marrow harvests.
  • Data collection on harvest duration, blood loss, cell yield, and complication rates.

  • Outpatient harvest duration (41 minutes) and hemoglobin changes were similar to inpatients.
  • Similar rates of packed erythrocyte transfusion were required for both groups.
  • Minor complications (hypotension, fever, nausea, pain) occurred in 11% of outpatients, with 9% requiring admission.
  • No serious complications were observed in the outpatient group.

Conclusions:

  • Outpatient marrow harvesting is a safe and feasible procedure for patients with hematologic malignancies.
  • The complication profile and resource utilization are comparable to inpatient procedures.
  • Outpatient marrow harvesting offers a viable alternative for selected patients.
