Canine lung allograft lymphatic alterations

I K Kline, P A Thomas

    The Annals of Thoracic Surgery
    |June 1, 1976

    Summary

Lung allograft rejection involves lymphatic obstruction, leading to fluid and cellular buildup that impairs lung function. This study observed dilated lymphatic channels, particularly in untreated recipients, indicating impaired lymphatic clearance during rejection.

    Area of Science:

    • Transplantation immunology
    • Pulmonary medicine
    • Surgical pathology

    Background:

    • Lymphatic obstruction is an underrecognized factor in lung allograft rejection.
    • Fluid and cellular accumulation, worsened by lymph stasis, compromises lung function post-transplant.

    Purpose of the Study:

    • To investigate the role of lymphatic alterations in lung allograft rejection.
    • To assess the impact of immunosuppression on lymphatic function in lung allografts.

    Main Methods:

    • Histological examination of lung specimens from dogs undergoing reimplantation or allografting (up to 133 days).
    • Evaluation of lymphatic channel morphology in untreated and immunosuppressed (azathioprine and prednisone) allograft recipients.

    Main Results:

    • Consistent finding of abnormally dilated perivascular, peribronchiolar, and subpleural lymphatic channels.
    • Dilated lymphatics were most prominent in untreated allograft recipients.
    • Immunosuppressed recipients showed attenuated lymphatic alterations, with impaired clearance of infiltrates and fluid.

    Conclusions:

    • Lymphatic obstruction is a significant feature of lung allograft rejection.
    • Impaired lymphatic function contributes to fluid and cellular accumulation, exacerbating rejection.
    • Immunosuppression may not fully restore lymphatic clearance capacity during rejection.
