Summary

Background:

  • Corynebacterium pseudotuberculosis is a known pathogen in domestic animals, particularly sheep, causing suppurative lymphadenitis.

Observation:

  • A previously healthy 34-year-old shepherd presented with a year-long history of a painful lymph node in the right groin.
  • Diagnostic cultures from the lymph node aspirate and tissue confirmed pure growth of Corynebacterium pseudotuberculosis.
  • Histological examination revealed suppurative granulomatous lymphadenitis with necrosis and bacterial clusters.

Findings:

  • This case represents the first documented instance of Corynebacterium pseudotuberculosis lymphadenitis reported in Spain.
  • The patient experienced a complete clinical recovery following surgical excision of the affected lymph node and a course of erythromycin.
  • Literature review indicated that most human cases occur in individuals with animal contact, primarily sheep, with Australia being a common reporting region.

Implications:

  • Corynebacterium pseudotuberculosis infection should be considered in the differential diagnosis of localized granulomatous lymphadenitis, especially in individuals with animal exposure.
  • This finding expands the known geographical distribution of human Corynebacterium pseudotuberculosis infections.
  • Highlights the importance of considering zoonotic origins for unusual presentations of lymphadenitis.
