Red cell alloimmunization after a bone allograft

Summary

Alloimmunization to non-ABO/Rh blood group antigens is rare after bone allografting. A patient developed anti-Fya and anti-Jkb antibodies post-bone transplantation, highlighting potential risks beyond expected blood group antigen systems.

Area of Science:

  • Immunology
  • Transfusion Medicine
  • Hematology

Background:

  • Alloimmunization to blood group antigens beyond the ABH and Rh systems is infrequently observed following bone allograft procedures.
  • Prior documented cases of such alloimmunization have predominantly involved the Rh system.

Observation:

  • A 20-year-old female underwent allogeneic bone grafting for osteosarcoma.
  • The patient had no history of blood transfusions or pregnancies.
  • Serologic testing revealed alloimmunization, with identified alloantibodies including anti-Fya and anti-Jkb.

Findings:

  • Fourteen months post-bone allograft, the patient developed antibodies against the Fya and Jkb blood group antigens.
  • This case demonstrates alloimmunization to less common blood group antigens after bone transplantation.

Related Experiment Videos

Implications:

  • Bone allografting can potentially lead to alloimmunization against a broader range of blood group antigens than previously recognized.
  • This finding underscores the importance of considering non-ABO/Rh antibody screening in patients receiving bone allografts, particularly those with potential for future transfusion needs.
