A patient was diagnosed with polyarteritis vasculitis, but autopsy revealed widespread microabscesses and tumor emboli from hidden colon cancer. This case highlights the need to consider alternative diagnoses that mimic vasculitis.

Background:

  • Vasculitis, a group of inflammatory conditions affecting blood vessels, can present with diverse and sometimes overlapping clinical features.
  • Polyarteritis (a form of vasculitis) is characterized by inflammation of medium-sized arteries.

Observation:

  • A 61-year-old male initially received an unequivocal diagnosis of polyarteritis vasculitis.
  • Post-mortem examination revealed no evidence of vasculitis.

Findings:

  • Widespread microabscesses were identified at autopsy.
  • Tumor emboli originating from an occult adenocarcinoma of the cecum were present.

Implications:

  • This case underscores the critical importance of considering differential diagnoses in suspected vasculitis.
  • Mimicking pathologies, such as disseminated infections or occult malignancies, must be thoroughly investigated to avoid diagnostic errors.
