Background:

  • Patients with chronic heart failure (CHF) experience exercise limitation.
  • This limitation is linked to an increased ventilatory response during physical activity.
  • Excessive dead space ventilation is a suspected contributing factor.

Purpose of the Study:

  • To investigate ventilation-perfusion (V/Q) matching during exercise in CHF patients.
  • To compare V/Q matching between CHF patients and controls with asymptomatic left ventricular dysfunction.
  • To determine if V/Q abnormalities explain the increased ventilatory response in CHF.

Main Methods:

  • Symptom-limited treadmill exercise was performed by 20 CHF patients and 8 controls.
  • Arterial blood sampling and expired gas analysis were conducted.

  • Fractional dead space ventilation and alveolar-arterial oxygen difference were calculated.
  • Main Results:

    • Fractional dead space ventilation decreased during exercise in both groups, more so in controls.
    • Alveolar-arterial oxygen difference increased in CHF patients but not controls.
    • Arterial carbon dioxide tension decreased similarly in both groups; arterial oxygen tension remained stable.

    Conclusions:

    • The similar changes in arterial carbon dioxide and alveolar-arterial oxygen difference between groups suggest V/Q matching is not the primary driver of increased ventilation in CHF.
    • Stable arterial oxygen tension further supports this conclusion.
    • The findings indicate that factors other than primary V/Q mismatch likely cause the exaggerated ventilatory response to exercise in chronic heart failure.
