Area of Science:

  • Gastroenterology
  • Minimally Invasive Surgery
  • Surgical Outcomes

Background:

  • Complicated cholelithiasis presents significant surgical challenges.
  • Assessing novel surgical techniques is crucial for improving patient outcomes.

Purpose of the Study:

  • To evaluate the feasibility and efficiency of laparoscopic cholecystectomy in patients with complicated gallstone disease.
  • To compare outcomes between successful laparoscopic procedures and those requiring conversion to laparotomy.

Main Methods:

  • Prospective study including 129 patients with complicated cholelithiasis (January 1990 - December 1992).
  • Patients underwent either successful laparoscopic cholecystectomy (Group I) or required conversion to laparotomy (Group II).
  • Data collected included procedure success rates, morbidity, mortality, and hospital stay.

Related Experiment Videos

Main Results:

  • Laparoscopic cholecystectomy was successful in 82% of cases (106 patients).
  • Group I (laparoscopic) had 4.7% morbidity, 0.9% mortality, and a mean hospital stay of 4.7 days.
  • Group II (conversion) had a significantly longer mean hospital stay (10.8 days, P=0.0001).

Conclusions:

  • Laparoscopic cholecystectomy can be safely performed in complicated cholelithiasis without increasing morbidity.
  • The procedure offers advantages of shorter hospital stays and faster recovery.
  • Careful patient selection and surgeon expertise are key to successful laparoscopic cholecystectomy.
