Background:

  • Coexistent acute aortic dissection and abdominal aortic aneurysm are exceptionally rare.

Observation:

  • A patient presented with acute severe back pain, diagnosed via CT scan with acute aortic dissection extending into a pre-existing 8 cm abdominal aortic aneurysm.
  • No signs of aortic rupture or leakage were observed.

Findings:

  • The patient was initially managed with antihypertensive medication for 2 months to stabilize the acute dissection.
  • Elective repair of the abdominal aortic aneurysm was performed with a novel technique: fenestration of the aortic septum at the proximal anastomosis to maintain perfusion of both true and false lumens.
