Summary

The Mitrofanoff procedure for continent urinary diversion demonstrated satisfactory clinical outcomes in 12 patients, achieving urinary continence for all during the day. This technique offers an effective continence mechanism with acceptable post-operative morbidity.

Area of Science:

  • Urology
  • Surgical Oncology
  • Reconstructive Surgery

Background:

  • Cystectomy necessitates urinary diversion.
  • Continent urinary diversion aims to restore normal urinary function and improve quality of life.
  • The Mitrofanoff principle offers a method for creating a catheterizable stoma.

Purpose of the Study:

  • To evaluate the clinical and symptomatic outcomes of patients undergoing cystectomy with Mitrofanoff continent urinary diversion.
  • To assess the efficacy and safety of the Mitrofanoff procedure in achieving urinary continence.

Main Methods:

  • A retrospective review of 12 patients (7 male, 5 female) who underwent cystectomy and Mitrofanoff urinary diversion.
  • Ileocecal segment used for reservoir; appendix or ileal tube as catheterizable conduit.

Related Experiment Videos

  • Outcomes assessed via modified Visick grading, with a median follow-up of 15 months.
  • Main Results:

    • No peri-operative deaths; 3 major early post-operative complications (lymphocele, abscess, ileus).
    • 4 patients experienced late stenosis requiring dilatation; overall symptomatic outcome satisfactory in 11 patients.
    • 100% daytime urinary continence achieved; median catheterization intervals were 4 hours (day) and 7 hours (night).

    Conclusions:

    • The Mitrofanoff procedure provides a satisfactory continence mechanism after cystectomy.
    • Despite a small sample size, the procedure shows potential for effective urinary diversion with acceptable morbidity.
    • Further studies with larger cohorts are warranted to confirm long-term efficacy and safety.
