Needle aspiration for suppurative post-BCG adenitis

Summary

Needle aspiration significantly improves outcomes for suppurative post-BCG adenitis. Aspirated lymph nodes showed higher regression rates and less sinus tract formation compared to controls.

Background:

Bacille Calmette-Guérin (BCG) vaccination is a common preventative measure for tuberculosis.
Suppurative adenitis is a known complication following BCG vaccination, presenting as inflamed lymph nodes.
Management of post-BCG adenitis often involves addressing lymph node enlargement and potential suppuration.

Purpose of the Study:

To evaluate the efficacy of needle aspiration in treating suppurative post-BCG adenitis.
To compare the resolution rates and complication incidence between aspirated and non-aspirated (control) groups.
To assess the impact of needle aspiration on lymph node regression and sinus tract formation.

Main Methods:

A prospective study comparing needle aspiration to conservative management for suppurative post-BCG adenitis.
Inclusion of 43 patients in the needle aspiration group and 34 patients in the control group.
Follow-up assessments at two and six months to evaluate lymph node regression and complications like spontaneous drainage.

Main Results:

Needle aspiration led to significantly higher lymph node regression rates: 58% at two months and 95% at six months, versus 9% and 65% in the control group.
Spontaneous drainage with sinus tract formation was markedly reduced in the aspirated group (7%) compared to the control group (44%) at six months.
The aspirated group demonstrated superior clinical outcomes in terms of node resolution and reduced morbidity.

Conclusions:

Needle aspiration is an effective and beneficial treatment modality for suppurative post-BCG adenitis.
This minimally invasive procedure accelerates lymph node regression and significantly lowers the risk of complications.
Needle aspiration should be considered as a primary therapeutic option for managing this condition.
