Summary

Inflammatory aortic conditions can be missed on Indium-111 leucocyte imaging. Awareness of the aorta as a rare uptake site is crucial for timely diagnosis and preventing serious complications.

Area of Science:

  • Nuclear Medicine
  • Vascular Imaging
  • Inflammatory Disease Diagnosis

Background:

  • Inflammatory aortic conditions often present with vague symptoms like fever.
  • Indium-111 labelled leucocyte imaging is used to detect occult sepsis or active vasculitis.

Observation:

  • A review of 1100 leucocyte scintigraphy scans identified only three cases with focal aortic uptake.
  • Initial interpretations mislocalized the aortic uptake to bowel, paravertebral abscess, or lumbar spine.

Findings:

  • The three cases involved periaortitis in Wegener's granulomatosis, aortic dissection in giant cell arteritis, and streptococcal aortitis.
  • Computed tomography (CT) and magnetic resonance imaging (MRI) were essential for correct diagnosis after initial misinterpretation.

Implications:

  • The aorta is a rare site for leucocyte uptake, increasing the risk of misdiagnosis.
  • Prompt recognition and further imaging are vital for managing life-threatening inflammatory aortic diseases.
  • Delayed diagnosis can lead to significant patient morbidity and mortality.
