Summary

Intraoperative mapping may not be essential for successful surgical ablation of ventricular tachycardia (VT). Encircling endocardial cryoablation shows promise even when intraoperative mapping is not feasible.

Area of Science:

  • Cardiology
  • Cardiac Surgery
  • Electrophysiology

Background:

  • Surgical ablation for ventricular tachycardia (VT) typically relies on pre- and intraoperative cardiac mapping.
  • Intraoperative mapping is not always possible, necessitating reliance on preoperative data.

Purpose of the Study:

  • To evaluate the necessity of intraoperative mapping for encircling endocardial cryoablation in ischemic VT.
  • To assess the contribution of intraoperative mapping to the success of VT ablation procedures.

Main Methods:

  • Retrospective analysis of 33 patients with inducible VT undergoing encircling endocardial cryoablation.
  • Patients were divided into two groups: those with (Group 1) and without (Group 2) successful intraoperative VT mapping.
  • Comparison of baseline characteristics and operative parameters between the two groups.

Related Experiment Videos

Main Results:

  • VT was inducible intraoperatively in 14 patients (Group 1) and not mappable in 19 (Group 2).
  • Reasons for failed mapping included noninducibility, nonsustained VT, polymorphic VT, VF, and incessant VT with hemodynamic compromise.
  • Operative procedures and outcomes were similar between groups, suggesting intraoperative mapping may not be critical.

Conclusions:

  • Encircling endocardial cryoablation is a viable option for ischemic VT, even when intraoperative mapping is not achievable.
  • The necessity of intraoperative mapping for successful VT ablation requires further investigation.
