Sleep disturbances are common in multiple sclerosis (MS), often caused by spasms or nocturia. Sleep apnoea is a less common but significant cause of nocturnal respiratory issues in MS patients.

Area of Science:

  • Neurology
  • Sleep Medicine
  • Medical Research

Background:

  • Sleep-related disorders are frequently observed in patients with multiple sclerosis (MS).
  • These disturbances can significantly impact patient quality of life and disease management.
  • The underlying causes and prevalence require further investigation.

Purpose of the Study:

  • To clinically evaluate sleep-related disorders in a cohort of multiple sclerosis patients.
  • To investigate the association between sleep disturbances and nocturnal oxygen desaturation.
  • To explore potential correlations between brain stem lesions and sleep-related respiratory issues.

Main Methods:

  • Clinical evaluation of 28 consecutive multiple sclerosis patients (12 males, 16 females) with varying disability scores (EDSS 1.5-8.5).

Related Experiment Videos

  • Patient-reported sleep problems including insomnia, leg spasms, snoring, and nocturia.
  • All-night oximetry to assess oxygen saturation (SaO2) dips, followed by polysomnography in select cases.
  • MRI analysis of brain stem regions.
  • Main Results:

    • 54% of patients (15/28) reported sleep problems, commonly leg spasms/discomfort and insomnia.
    • Three patients exhibited significant nocturnal oxygen desaturation (> 5 dips of > 4% SaO2/h).
    • Polysomnography confirmed sleep apnoea in 2 of these 3 patients. MRI revealed brain stem abnormalities in 20/22 patients, with no clear correlation to nocturnal desaturation.

    Conclusions:

    • Sleep disturbance is a common and often under-recognized issue in multiple sclerosis.
    • While typically linked to spasms, pain, or nocturia, sleep apnoea can cause nocturnal respiratory insufficiency.
    • Brain stem lesions are prevalent in MS but did not directly correlate with observed sleep apnoea in this study.
