Summary

This study reveals distinct anatomical and functional models of choroidal lobules. The observed lobular patterns in angiography differ from the actual anatomical structure of the choriocapillaris.

Area of Science:

  • Ophthalmology
  • Vascular Anatomy
  • Microscopy

Background:

  • The choroidal angioarchitecture, specifically the choriocapillaris (CC), is a subject of debate between anatomists and clinicians.
  • Understanding the CC's structure is crucial for diagnosing and treating various ocular conditions.

Purpose of the Study:

  • To resolve the controversy surrounding the choroidal angioarchitecture.
  • To differentiate between anatomical and functional models of choroidal lobules.

Main Methods:

  • Utilized scanning electron microscopy (SEM) on vascular casts from 36 human and 10 Rhesus monkey eyes.
  • Analyzed the patterns and structures of the choriocapillaris from peripapillary to peripheral regions.

Main Results:

  • The choriocapillaris (CC) exhibits non-homogenous patterns varying with location.
  • Anatomical lobules of the CC are not identical to the functional lobules observed in angiographic studies.
  • Collecting venules were found in 86% of anatomical lobules, while feeding arterioles were present in only 14%.

Conclusions:

  • Two distinct models of choroidal lobules—anatomical and functional—should be recognized.
  • The lobular appearance in fluorescein (FA) and indocyanine green (ICG) angiography is likely due to blood flow pressure gradients.
  • SEM analysis provides a clearer understanding of the true choroidal vascular structure.
