Summary

Subacute sclerosing panencephalitis (SSPE) is more common in males and linked to measles infection in unvaccinated children. Measles antibodies are significantly higher in SSPE patients

Area of Science:

  • Virology
  • Neurology
  • Epidemiology

Background:

  • Subacute sclerosing panencephalitis (SSPE) is a rare, progressive neurological complication of measles virus infection.
  • It primarily affects children and young adults, leading to severe neurological deficits and often death.

Purpose of the Study:

  • To investigate the epidemiological and virological characteristics of SSPE cases in Delhi.
  • To analyze measles virus antibody titers in serum and cerebrospinal fluid (CSF) of SSPE patients.

Main Methods:

  • Retrospective analysis of 65 SSPE cases referred to the Virology Laboratory, National Institute of Communicable Diseases, Delhi (1989-1992).
  • Collection of demographic data, clinical history (including measles exposure and vaccination status), and serum/CSF samples.
  • Measurement of measles virus-specific antibodies using hemagglutination inhibition (HAI) assays.

Main Results:

  • SSPE cases (n=65) were predominantly male (6:1 ratio) and aged 2-24 years.
  • A history of measles infection was reported in most patients under 3 years; no SSPE patients had a history of measles vaccination.
  • Significantly elevated serum and CSF measles HAI antibody titers were observed in SSPE patients compared to controls.

Conclusions:

  • The study reinforces the association between measles virus infection and SSPE, particularly in unvaccinated individuals.
  • The findings highlight the importance of measles vaccination in preventing SSPE.
  • Elevated measles antibody titers in serum and CSF are indicative of SSPE.
