    Summary

    Photochemotherapy (PUVA) treatment did not alter prostaglandin biosynthesis in psoriasis patients. Studies found no significant changes in prostaglandin levels before, during, or after PUVA therapy for recalcitrant psoriasis.

    Area of Science:

    Background:

    • Psoriasis is a chronic inflammatory skin condition.
    • Prostaglandins (PG) are implicated in inflammatory processes.
    • The therapeutic mechanism of PUVA for psoriasis remains incompletely understood.

    Purpose of the Study:

    • To investigate the effect of PUVA therapy on prostaglandin biosynthesis in patients with recalcitrant psoriasis.
    • To determine if changes in PG levels correlate with the therapeutic response to PUVA.

    Main Methods:

    • Biopsies were obtained from 9 patients with recalcitrant psoriasis before, during, and after PUVA treatment.
    • Prostaglandin biosynthesis was measured using PGE1-equivalents.
    • Incubation with exogenous arachidonic acid was performed to assess enzyme activity.

    Main Results:

    • No statistically significant differences were observed in prostaglandin concentrations in involved or uninvolved psoriatic skin before, during, or after PUVA treatment.
    • Prostaglandin levels in involved skin ranged from 2.01 to 2.85 ng/g wet weight, and in uninvolved skin from 2.23 to 3.29 ng/g wet weight.
    • The addition of arachidonic acid increased PG activity, but no significant differences were found between treatment phases.

    Conclusions:

    • The hypothesis that PUVA stimulates prostaglandin biosynthesis to mediate its therapeutic effect in psoriasis is not supported by these findings.
    • PUVA treatment does not appear to influence prostaglandin biosynthesis in recalcitrant psoriasis patients.
