Peripherally induced tremor and parkinsonism

Summary

Peripheral nerve injury can cause tremor and parkinsonism. Central nervous system reorganization following trauma may lead to these movement disorders, even spreading beyond the initial injury site.

Background:

Peripheral nervous system trauma is a known cause of dystonia and tremor.
Peripherally induced parkinsonism has not been previously documented.

Observation:

A study reviewed 28 patients with movement disorders linked to local injury.
Tremor was present in all patients; 11 had additional parkinsonian features.
Movement disorders spread beyond the initial injury site in 20 patients.

Findings:

Severe local injury preceded movement disorder onset.
Predisposing factors, like essential tremor, were noted in some patients.
Tremor was unresponsive to medication; parkinsonism showed limited response to levodopa.

Implications:

Central reorganization following peripheral injury may cause motor disturbances.
This phenomenon can manifest as tremor and parkinsonism.
Further research into mechanisms of peripherally induced movement disorders is warranted.
