Area of Science:

  • Pharmacology
  • Gastroenterology
  • Psychiatry

Background:

  • Clozapine is reserved for treatment-resistant schizophrenia due to the risk of agranulocytosis.
  • Potential side effects of clozapine may be underreported due to its restricted use.

Observation:

  • A male schizophrenic patient developed agranulocytosis.
  • Following agranulocytosis, the patient subsequently developed neutropenic enterocolitis.

Findings:

  • This case represents the first reported instance of neutropenic enterocolitis associated with clozapine-induced neutropenia or agranulocytosis.
  • The study reviews clozapine's history, side effect profile, and the pathophysiology of neutropenic enterocolitis.

Implications:

  • Highlights a previously undocumented severe adverse event linked to clozapine therapy.
  • Suggests the need for increased vigilance regarding gastrointestinal complications in patients on clozapine, especially those with neutropenia.
