Whipple's disease: case report with immunological studies

    Summary

    Whipple's disease may be caused by a bacteria-like agent sharing antigens with streptococci and Shigella flexneri. The patient's T cell function was normal, suggesting T cell dysfunction isn't required for this condition.

    Background:

    • Whipple's disease is a rare systemic disorder of unknown etiology.
    • Previous hypotheses suggested an infectious cause, but definitive proof remains elusive.

    Observation:

    • A patient with Whipple's disease underwent small bowel biopsy.
    • Immunofluorescence techniques were used to test tissue antigenicity.
    • Lymphocyte response to mitogens was assessed.

    Findings:

    • The patient's biopsy showed antigenicity to specific streptococci (groups A, B, C, G) and Shigella flexneri.
    • No antigenicity was found with pneumococcus or Shigella sonnei antisera.
    • The patient's T lymphocytes responded normally to phytohemagglutinin and concanavalin A.
