Gastric carcinoids. The Yale Experience

Summary

Gastric carcinoid diagnoses are rising, likely due to increased endoscopies, not incidence. Multiple tumors with hypergastrinemia show non-aggressive behavior, suggesting conservative surgery may be suitable.

Area of Science:

  • Gastroenterology
  • Oncology
  • Pathology

Background:

  • Gastric carcinoids are rare neuroendocrine tumors.
  • Increased incidence may be linked to diagnostic advancements and low acid states.
  • Chronic atrophic gastritis is a common comorbidity.

Purpose of the Study:

  • To document experience with gastric carcinoids over a decade.
  • To identify lesion frequency and association with low acid states.
  • To analyze pathobiological characteristics and survival outcomes.

Main Methods:

  • Retrospective case series of 16 patients.
  • Evaluation over a ten-year period at a tertiary care center.
  • Therapies included gastrectomy, polypectomy, and endoscopic surveillance.

Related Experiment Videos

Main Results:

  • All carcinoids originated in the gastric fundus.
  • Chronic atrophic gastritis was present in 63% of patients.
  • Ten of thirteen patients survived with a mean follow-up of 4.7 years.

Conclusions:

  • Increasing gastric carcinoid diagnoses may reflect heightened awareness and endoscopy rates.
  • Malignant progression prediction criteria are lacking.
  • Multiple gastric carcinoids with hypergastrinemia often exhibit non-aggressive behavior, supporting conservative management.
