Replacing empty ganciclovir implants in patients with acquired immune deficiency syndrome (AIDS)-related cytomegalovirus (CMV) retinitis can preserve good vision. Most patients maintained visual acuity of 20/40 or better after repeat ganciclovir implant procedures.

Area of Science:

  • Ophthalmology
  • Infectious Diseases
  • Virology

Background:

  • Cytomegalovirus (CMV) retinitis is a common opportunistic infection in patients with acquired immune deficiency syndrome (AIDS).
  • Ganciclovir implants provide sustained local delivery of antiviral medication for CMV retinitis.
  • Implant replacement may be necessary due to ganciclovir depletion.

Purpose of the Study:

  • To evaluate the surgical experience and outcomes of replacing empty ganciclovir implants in patients with AIDS-related CMV retinitis.
  • To assess the efficacy and safety of repeat ganciclovir implantations.

Main Methods:

  • Retrospective chart review of patients who received two or more ganciclovir implants.
  • Analysis of patient demographics, treatment history, visual acuity, and CMV control.

Related Experiment Videos

Main Results:

  • Nine eyes in eight patients underwent ganciclovir implant replacement, with an average interval of 6 months between implants.
  • CMV retinitis was controlled in all but one patient.
  • Five of eight patients maintained visual acuity of 20/40 or better, despite long-standing CMV retinitis.

Conclusions:

  • Repeat ganciclovir implant surgery is a viable option for managing CMV retinitis in patients with AIDS.
  • Most patients can retain good visual acuity following ganciclovir implant replacement.
  • Some patients may still require systemic anti-CMV therapy.
