Summary

Pseudoaneurysm formation is a rare complication after aortic and renal artery bypass surgery. This case highlights a unique contained leak from a renal artery stump causing vena caval issues.

Background:

Simultaneous aortic and renal artery revascularization is a complex procedure.
Pseudoaneurysm formation is a known, albeit uncommon, complication, typically at anastomosis sites.

Observation:

A patient presented with symptoms after abdominal aortic aneurysmectomy and bilateral aortorenal bypass.
Imaging revealed an unusual contained leak from the proximal renal artery stump.

Findings:

The renal artery stump leak caused inferior vena caval compression.
Associated inferior vena caval thrombosis was observed.
Computed tomography and angiography were crucial for diagnosis.

Implications:

This case expands the understanding of pseudoaneurysm complications in aortorenal bypass.
Highlights the importance of vigilant surveillance for rare complications.
Informs surgical technique and postoperative management strategies.
