    Summary

Palliative urinary diversions offer a survival of 187 days for malignant ureteral obstruction. While renal function improved in many, high morbidity and mortality were noted, with most patients returning home.

    Area of Science:

    • Urology
    • Oncology
    • Nephrology

    Background:

    • Malignant ureteral obstruction presents significant challenges in palliative care.
    • Urinary diversion is a potential intervention for such cases.

    Purpose of the Study:

    • To analyze the outcomes of palliative urinary diversions in patients with malignant ureteral obstruction.
    • To identify factors influencing survival and patient selection criteria.

    Main Methods:

    • Retrospective analysis of 62 patients undergoing palliative urinary diversion.
    • Evaluation of postoperative survival, renal function, morbidity, and mortality.
    • Assessment of patient selection criteria and operative procedure choices.

    Main Results:

    • Average postoperative survival was 187 days.
    • Renal function normalized in 64% of azotemic patients.
    • High morbidity and mortality rates were observed, linked to underlying disease and adjuvant therapy.
    • Nearly two-thirds of patients were discharged, spending 84% of remaining survival time at home.

    Conclusions:

    • Palliative urinary diversion can improve quality of life by enabling patients to return home.
    • Patient selection and operative procedure choice are critical for optimizing outcomes.
    • Survival is not strongly influenced by tumor characteristics or prior treatments.
